Appointments

To schedule an appointment, please call us at (360) 876-5440 between the hours of 8:00 am and 5:00 pm Monday through Friday.

We offer same-day sick visits and appointments for patients with urgent problems to avoid ER visits, when possible.

New Patients

If you're a new patient with us, please arrive at least 15 minutes early to your first appointment to complete the necessary forms for enrollment.

Medication Refills

Please call your pharmacy for refill request and allow 48-72 hours for any medication refills to be processed.

What to bring to your first appointment

Your current insurance card and picture ID.

Your current medications, including any over-the-counter medications or supplements, or a list of them including dosage information.

Questions you have for your physician regarding your health concerns.

Prescription refill requests.

Co-Pays/Bad Check Charge

All co-pays are due at the time of service. If for whatever reason you were to have insufficient funds to cover a check you wrote to us, an additional charge of $45 will be assessed to you to cover the costs.
